The Book of Scale (which is given to the Oracle for the phylactery) originally resulted from a turnin in Neriak (torn, burnt book + torn, frost covered book + 1000pp).

The Oracle of K'arnon has two philacteries associated with him: The new one from this quest (that we still don't know much about), and Miragul's Phylactery, a quest item for the paladin epic.
Here's a very short writeup of this step of the paladin epic, for the curious. This has nothing to do with our current quest.
To obtain the quest item, you had to obtain the torn, burnt book and the torn, frost-covered book from either Nagafen and Vox (respectively) or from a number of Kunark dragons. You would then take them to a scribe with a fee of 1000pp, who would combine them into the Book of Scale. Handing the Book of Scale to the Oracle gives you Miragul's Philactery, a no-drop item with no stats. You then find the Lich of Miragul in Everfrost, charm him, and hand him the Phylactery, which spawns the live Miragul. You quickly remove the "live" qualifier from his name, then collect his head and robe. These two items, plus others, are turned in for the Fiery Avenger, an interim step in the paladin epic.
The scribe I mentioned earlier was originally the bartended at the Blind Fish tavern, which is what the reference in the note is all about (and in fact, I think you can still combine the Book of Scale with her). Eventually, then-Verant realized that paladins don't belong in Neriak, especially not when doing a quest for their most holy blade. So they made a new NPC scribe outside Qeynos, though you would only discover that she is a scribe if you chatted with a particular person in Lake Rathetear -- he would give you the key words to make her talk.
I can also confirm that Miragul drops nothing but the two quest items; I just killed him a couple of days ago (after this quest went live).
