I made one of these last week. The hard parts are the ivory skull and the mephit blood. I had to buy my blood from the bazaar. The helm has no graphic. The icon is the old grotesque mask icon.
Drops:
Ivory skull from any mob in the Lair of Terris Thule (rare drop).
Lock of Banshee Hair from banshees in P o Nightmare.
Midnight Stones from a lot of mobs in P o Nightmare.
Nightmare Mephit Blood from mephits in P o Nightmare (difficult to get to).
Brick of Nightmare Iron from Hobogoblins in P o Nightmare.
On Tunare, this item would sell for around 20k I believe.
