SOE have stated that they are currenly unable to give an item more than one %modifier.
Not sure why, not sure how.... some kind of coding restriction i guess.

It is also blatently incorrect with the new aug system in place. The way it would work is like this. You place an NPC in PoK. You set him up so that anyone can get a base item via a triggered text. That item would be a jack-of-all-trades item with 7 empty slows. Your slot type would be a new type for tradeskill mod augs, say type 9. Each of the 7 slots would be of that type. Now, on a turnin, you turn in your trophy plus your highest %modifier item for that skill that you have. The npc checks your skill to see that you are at 250, if you are not he hands the items back and says you need to work on your skill a little more. If you do, he hands you an aug for that skill equivalent to whatever the highest mod that you gave him was. So if you gave a 15% smithing mod item with your tradeskill trophy, that's what the aug for smithing would have. You then take that and the base item and combine in the aug pool. Collect all 7 and you have a snazzy item to show people to represent what skills you have 250 in, and need only equip one item when tradeskilling.
