I believe the problem in instances was with the scripts for trophies not loading properly to evolve them on successful combines and was unrelated to skillups. Thats a core code of the gamer as far as I know and unhindered by whether youre in an instance or not.

If you can make the combine at all, then all NORMAL parts of a combine will work including skillups, successes, failures, failure returns, AA's, etc.
Scripts are a separate file, not a normal part of the combines, and can fail separately. Trophy evolution is a script.
if for some reason you cannot at all make a combine that should be working in an instance, move out of the instance. If the combine then works the instance was missing recipe data. If the combine still does not work, check your combine very carefully. If it still does not work, try a very simple very common no fail combine, if that does not work, then recipes are broken overal.
It is much harder to tell in the guild hall if scripts are failing. The only other thing there that relies on scripts it he teleporter, and then you can only tell it works if it successfully teleports you out.Ngreth Thergn

For some skills like smithing where an alternate stat besides wis/int can count, you can get a shaman to buff your str above the cap and the buffs do not fade in the guild hall. Also, there is easy access to a bank and rarely any competition for a tradeskill container.
I personally like to tradeskill where it is most convient for me. For smithing I like to work in CR and or the bazaar depending on if I need my alt to help hold things or do any combines. For tailoring, PoK is also real nice for tradeskilling as it has most everything (almost) and is much closer to the bazaar in case I need to go get something thats not on a vendor.

bad runs?
I realize that sometimes I'll have a bad run, but how many runs do I have to have before I get a good one? Or even an average one?
With 440 wisdom, at 280 skill, chance to get a skill gain is 4.1%. With 160 combines (I didn't write down which combines I was doing for this run), I got 2 skill gains (both from elaborate, I think there was 30).
At 282 skill, chance to get a skill gain is 3.94%. With 46 no-bonus combines, 63 intricate/sacred combines, and 29 elaborate combines, I got 3 skill gains, all 3 from elaborate.
At 285 skill, chance to get a skill gain is 3.7%. With 16 intricate/sacred combines and 100 eminent combines, I got 7 skill gains, all from eminent.
Nice to finally get some gains, but if the bonus for eminent is truly +10%, that's still low. So I've tapped out what I've been saving since templates went in, anything under 1k each in the bazaar, and even what a sweet darling caster was willing to give me despite needing to work on her own tailoring. Only superb quality materials are giving me skill gains, and every source I've heard of is dropping flawless now. So basically I can look forward to another 4 months of collecting materials before I can do another run, get 3 skill gains, and throw my computer through a window. *sigh*

The predicted skill up rate at the higher end gets up to 40-50 combines per point. Again this is an average and some will be higher and some will be lower.
I did over 500 combines from 277 to 289 mostly on templates. I then did an assortment of templates but a lot more sacred chest/leg augs. Ended up having 298, 299, and 300 come back to back to back on sacred symbols. It is all over the place and persistence is key.
