Just from what I have read, you need to completely finish the Bertoxx quest which means you have to kill quite a few nasties in PoP.
You gotta have PoNb, which leads to Teris thule, after you kill her then you gotta go to plane of torment and kill keeper of sorrows and Saryrn, then you gotta kill grummas for crypt of decay, then do the carpin cycle and kill him to get flagged for bertoxx's lair, then kill him.

I suppose it might simply be that you need to kill bertoxx only, but that at the very least requires the carpin cycle.
Basically, if your not in a large raid guild, or are at least friends with someone who can get you in those raids, you are screwed.
Oh BTW, get level 60+ before you worry about any of that stuff
