I was working on my tailoring skill ups doing the small/Epicthieck's power Belts. I have tailoring mastery 3 and the Akhevan Shadow Shears that have a 15% mod. With the shears alone my tradeskill lvl shows 260-269 from start of session to close today. Tailoring mastery 3 should obviously be giving that an even higher bump to were chance of success of combine is at the max 95%. Yet I failed rougly 35% of my combines over the last 2 days and 140 combines or so. Is there a bug with skill checks with mods and AA's?

Some items have fixed success rates (like the DON cultural armo). I would take the reply from Ngreth to mean that Epicthieck's Power Belts do as well.

They certainly do. The Prepared Sporali Skin is also a fixed rate as I fail about 40% of them, and they are trivial to a 336 skill.Brantes Bonetinkerer

Yup, they surely do have a minimum fail rate.
That is the slight problem with sites like this, it separates it from the in game book that tells you how to do this combine, in which it reasonably clearly points out that even those with the best skill will fail often
