HELP: Phantom Shadow (65 Monk)

Has anyone determined the recipe for the level 65 monk tome: Phantom Shadow? I've tried many combinations involving Debilitating Thickener, Ralos' Ink Additive or Ink Additive of the Nameless, and 1-3 total inks of either Obstruction, Myopsis, and Tranquility along with a Quill of the Transcendent and Runic Parchment Manual but have not had any luck yet.

Broken?

Enchanter line (Pacify, Pacification) with same 3 effects only uses a single Ink of Tranquility. But that doesn't work here. My guess is .... broken.
