Let me preface this question by saying I haven't spent a lot of time in BoT.

The Adv. Combat manual quest calls for a sandstorm gem. is this a different drop than the sandstorm pearl? And if so is it from the same mobs?

Sandstorm Gem is NODROP. Its the same item which you need for courtyard key.
On Karana server, there are always a lot of groups in BoT, which will let you loot any of nodrop gems, since most ppls either have key or dont need it.
