I can't find the text at the moment, but the NPC is Grennik Neltrin in Gulf of Gunthak (or however you spell it) for the spell Pure Blood (removes 18 poison and disease counters)
In a brew barrel combine A Blackened Vial (drop in Nadex) + the troll skull blood (drop in Nadex off the Blackhands) + Gunthak Gulf Water (Foraged and dropped in Gunthak). You get A Black Potion.
Then combine A Black Potion + Pure Blood (dropped off a Luggold Initiate in Nadox) to recieve A Glowing Black Potion.
Turn in A Glowing Black Potion to Grennik and receive the spell Pure Blood, which is no drop.

The folks who have reported on this have all been brewers and have all reported the brewing combine to be trivial (lowest report is trivial at 167). Our guess is that it is a no fail combine.
But, it's a quest that involves a tradeskill, so I thought you might want to know.
