Which means you cannot equip, or use the item until you meet or exceed the specified Level.
Recommended Level: XX
Which means you can equip and use the item (if you meet the Required Level), but the various other attributes the item modifies, like Strength, Dex, Resistance, etc... will not be at the highest level until you meet or exceed the Recommended Level. You will get a portion of the bonus, which increases as you get closer to the Recommended Level.
If an Item has both a Required Level and a Recomended Level, the Required Level should always be less than the Recomended Level.

This is intended to provide tradeable items that won't over-power lower level characters, but the characters can "grow" into them. Just another way to prevent excessive twinking, without making everything NO DROP.

And to answer the next question that is undoubtedly coming. The effect, Flowing Thought 1, works starting at level 1. In fact, it's really great at low levels, as that is an appreciable amount added to your mana regen. At high levels, one more or less mana regen is not particularly noticable, but, the stats aren't bad, and FT adds up.
