  • 200 over trivial/no fail broken?

I think there was a rule for very trivial items to be no fail for high level tradeskillers. With a skill of over 200 more than the item's trivial, the combine would no longer fail. If this was correct, it is broken now. I was combining Drop Of Mercury (62 trivial) with a skill of 251 + 5% = 263 and failed one.

I believe the no-fail check only looks at your raw skill, not your modified skill. At skill 251, anything trivial 50 or less will be no fail for you.

Would be an explanation, yes. Still, not many checks look for the raw skill, most use the modified skill. Other possibility is that the no fail check only worked for trivials 51 or less and they forgot to update that.

I've had a 250 raw skill trader fail on things with level 16 trivials this week. They broke this.

I wasn't aware there was such a thing as a no-fail combine aside from the stuff marked with a trivial of 15.

Once you exceed a trivial by 50 raw skill your success chance goes to 96.25
          100 above trivial is 97.5
          150 above trivial is 98.75
          200 above trivial is 100%
(at least this was true before the DoN patch)
The calculater on the main site reflects these rules.

OK, this suggests that something changed in the no-fail success system. It may or may not be intentional. I would suggest /bugging it just to be sure.
