I discovered the hard way that there is a feature (no - they tell me it isn't a bug!) that when you craft a lore item and have one in your possession (inventory or bank) the combine will work, but the item created will poof!
In my case I was using up leftover materials from my 8th shawl smithing, had an Engraved Royal Velium BP in inventory, and did the final combine on another:
[Sat Feb 22 10:31:15 2003] You have fashioned the items together to create something new!, and
[Sat Feb 22 10:31:15 2003] Duplicate Lore Items are not allowed.
Jewelry Kit contains only engraving tools, nothing on the ground or in inventory.
By means of intense nagging, I have obtained a GM promise to restore the Imbued Royal Velium BP "if possible", with a note that this is a "one time exception"
It would seem that this would be something that is simple to patch, but ...
In the interim, let the crafter beware.

Didn't even think to check it, even though I had a Black Acrylia Halberd poof on me from blacksmithing (though that was months ago)
