Are there working instructions on how to add an avatar?

I checked the faq, but its unavailable the last few days.

I went under User CP, but Im not able to get it to stick.

First: the obvious, visitor are just visitor and likely wont get the privilage of avatar among other things so you'll need to sign up for membership. It's free so what are you waiting for? =)

Once you have created a member account:
Go to User CP
Click on Edit Avatar on the left side bar
Select 'Use Custom Avatar'
Enter the web site address *OR* browse for a file on your hard drive. Max is 100x100 and 19.5k so if your avatar is bigger, you will need to edit them down a bit.

Finally Click on Save Changes button.

I dont know if the forum member have to have certain post count or be on forum for so long.

"Visitor" is just the newbie forum title, after some undisclosed # posts, that will change.

Hmmm... I stand corrected. I didn't know visitor was a member title.
