Can anyone confirm failing on Mithril Arrow shafts?(renamed)

Sat down, gathered materials for 100 Arrow combines (5 stacks)combines.

212 fletching skill
265 dex

0 failures on mithril shafts (is this a no fail combine?)
2 failures on fletch combines
15 failures on Arrow Head Combines (donno whats up there)
Yield: 5 stacks of components each

100 combines later
22 Successful combines
0 skillups.

now that I think about it the LAST set of Mithril shafts I made before this recent fubar was 100% success rate too. Can anyone confirm they've gotten a failure on Mithril Arrow Shafts?

I did mithril for a bit, but it was a while back. But, I don't remember failing on the shafts...hmm.

Out of 100 combines on the Mithril shafts I failed once. so Yes you can fail but not often

Hmmm...I seem to be the odd one. When I do fail, I fail on the mithril arrowshafts, not much else. I have never failed on the fletches, and I fail on the arrowheads rarely. But the shafts seem to be the problem for me. They are definitely not no-fail, although my smithing is only 48.
