ok this is updated a lot, but a few questions:

What is the loc of Siska the Spurned in Dulak's Harbor?
"obtained from Kithikor in Kithikor Forest" How is it obtained? Kill, Quest, etc?
What are the two halfling full names at the chessboard in PoM, and thier locs please if possible? Also, obtained how? Quest, kill?

Can this only be combined in a normal Forge? Can only a rogue do the final combine?

Sorry, I totally forgot about this. Here are your answers.

What is the loc of Siska the Spurned in Dulak's Harbor?

Loc should be neg 720, neg 525. In the tavern at the bar.

"obtained from Kithikor in Kithikor Forest" How is it obtained? Kill, Quest, etc?

Loc for Kithicor is neg 555, pos 1960. Keyword is "blue orb." No killing needed.

What are the two halfling full names at the chessboard in PoM, and thier locs please if possible? Also, obtained how? Quest, kill?

Ronald Brumplefoot or Leea Brumplefoot. Both are supposed to work. Keyword is "blue orb". No killing needed.

Can this only be combined in a normal Forge? Can only a rogue do the final combine?

Personally, I attempted this in PoK forge. Items are notrade, and only obtainable by a rogue, so I would guess only a rogue can do the combine.
