The Yield: 3 recipe includes the wrong size of ore, and should be using Large Brick. I would assume the Yield 6 should be using a Block as well, but haven't tried to confirm.

I assume you are correct about the 6-yield combine.
I'm not sure how we got the wrong information into the database, but I've made both these corrections and they should be showing properly now.
