This item also drops in Plane of Hate. Unfortunately I didn't make note of exactly who dropped it. It was either the zombie mobs or the mummy mobs (Undeads).

Dropped or me last night off of a malevolent banshee. Did not know what it was until I came here. Only saw one drop in a 4 hour killing fest though.

Actually, Alla's has the drop listed on the banshee already, so that must have been it.
