seems to be no-fail. a guy with smithing skill of 1, 2, and then 3 and succeeded on all (and got skill-ups for all...). i know that "no-fail" is hard to prove, but could we get a note on the combine that it's suspected?

As I recall, anything trivial 15 (or lower) is no-fail. If you want to test this on the vial, then have someone take 1 minute to skill up to 15 before combining. If they get the can no longer advance skill message then it must be no-fail.
As a side note, Solstice earring is probably trivial 15 too.Master Artisan Kahmon

Sounds good to me, updated the vial. The final quest combine in the forge in the Solstice quest doesn't result in the Earring of the Solstice. It results in Xanthe's Earring of Nature and the Protection of the Cabbage spell, both of which are already marked No Fail. (You then turn in Xanthe's earring if you want the Earring of the Solstice.) While a trivial of 15 does mean a recipe is also No Fail, we don't assign that trivial to it unless the trivial has been pinpointed at 15 (either by narrowing the range or by seeing it in the UI). If we conclude that it's No Fail before it's pinpointed at 15, we just mark it No Fail and are done with it.
