I'd assume it's the same as the normal recipe for multi-valued potions:

5 or 10 x YOUR_POTION_HERE in [foldable] bearskin potion bag, NO FAIL combine, shaman only.

That should read: "5 or 10 x YOUR_POISON_HERE in coffin poison bottle, NO FAIL combine, rogue only, also returns 5 x advanced poison vials"

Well we really can't take "assumed" recipes... it may need a Coffin Poison Bottle, or something else... it would be better to have someone actually test it out for the recipe.
