Proper Recipe is as follows, I did this myself:
Pot + Pondfish Spine + Pondfish Spine + Gray Colored Ice in Oven. Trivial < 190, Returns Pot, Poison Spine Extract.
Flask Of Water + Bottle + Hopeless Willow Moss + Poison Spine Extract in Brew Barrel. Trivial < 190, Returns Grayish Liquid.
Both Combines trivial less than 190 skill (Yes, both my brewing and baking are 190 exactly) Both resulting items Identify the same as their name. This final result is handed in to a certain froglok for a Iksar warrior quest which you get from the named Skoripco by in the Overthere, at the entrance to the canyon.
In teh current database entry, the recipe is incorrect. While I can not provide more accurate trivials it seems to be around 45, based on various people who have tried.
