3*Purified Water=Water Reed+Purification Tablet combined in Tracker's Water Extractor
The Water Reed is foraged in Lake of Ill Omen. The rumor threads from Allakhazam.com suggest that it is foragable in many other zones as well. The same rumor mill is saying that other foraged items can be substituted for the Water Reed such as Moss Root.
The Purification Tablet is obtained from NPC vendors in West Freeport(Toxdil and the Brewing vendor near the North Freeport zone line), Field of Bone (both personally confirmed), Lake of Ill Omen, and Warsilik's Woods. The Purification Tablet is LORE.
The Tracker's Water Extractor is obtained from the same merchants as the Purication Tablets in the zones near Cabiliss. These merchants seem to only stock one of extractor per spawn. I purchased my extractor from Klok Acet in Field of Bone. He had only one of them in his inventory with standard items, such as fishing pole and bait, below the extractor. I can only think of one other time where an NPC has not stocked unlimited quantities of an item that is generated by the NPC vendor: Crows Special Brew with maroon bottle icon(not Crow's Special Brew with green bottle icon) from crow in Qeynos.
