Been doing tradeskills for six months and had a chanter who couldn't get to 199 from 198 in jewelcraft after cruising to 198. Putting this together with other hell level posts it seems that hell levels are actually two RNG events (RNG stands for Random Number Generator).
The RNG rolls on each combine, of course, but the most rational statistical explanation is that the RNG also sets the degree of difficulty for your next skill up. Some get a bad roll and a high degree of difficulty for the next skill up and some don't. The chances of getting a higher degree of difficulty increases in the known hell levels, which is why the're hell levels.
If this is true the difficulty RNG roll could occur at the toons birth or more likely when a skill up is achieved the degree of difficulty is set for the next skill up.
