Question(s) about illusion potions

First of all I am not a shaman but am trying to look up as much info so I can tackle a shammy and ask him to make me illusion potions.

My questions are these,

What does a Shaman need to be able to make a illusion potion (apart from the required skill that is)

For example, I want DE potions, I know I need DA parts and that wormwood component.
Now do I need 1 DA parts and 1 Wormwood to make a 1 shot potion or is more required ?
And is it possible to make 10 shot DE illusion potions ? , if so... does that shaman need 10 DE parts and 10 Wormwood for it ?

The ingredients - the relevant racial meat and wormwood

That's correct, one of each.

Yes. Assuming all the combines succeed. It is ten individual single potion combines, then a ni fail combine to turn it into a ten shot.

I've been unable to combine the illusion potions into 5 and 10 shots. No idea if this is just me or everyone.

I have been able to 10 shots but not 5.

I'm told the fixes for pre-LoY combines in folded medicine bags, and all nonfunctional combines in folded bearskin bags, should be fixed as of the next patch.
